Once you have completed the form, click Submit.  You will then need to either mail the entry fee (by check) or drop off entry fee to Copy Systems in the Destin WalMart.  You can also print out the attached Chili Entry Form, sign and mail/drop off with fee.

 If entry is for a non-Parrot Head, you will also need to complete an application form in the name of one person and send/drop off with $45 entry fee.

The participant named hereby voluntarily releases, discharges, waives and relinquishes any and all actions or causes of action for personal injury (including death) or property damage occurring to himself/herself arising as a participant in the said program or any incidental activities. The participant named agrees that under no circumstances will he/she or his/her heirs, executors, administrators, and assigns prosecute or present for, or on behalf of, and of their activity officers, employees or volunteers, for any of said or similar causes of action, including those which arise by the negligence of the Emerald Coast Parrot Head Club, Fudpucker‘s of Destin or of any said persons, whether passive or active. It is the intention of this instrument to exempt and release the Emerald Coast Parrot Head Club and Fudpucker‘s of Destin from liability for personal injury, property damage or wrongful death alleged or caused by negligence.

The participant named further agrees to defend, indemnify and hold harmless the Emerald Coast Parrot Head Club and Fudpucker’s of Destin, their officers, directors, employees and volunteers, from any claims, demands, damages, costs, expenses or liabilities arising out of his/her participation and/or the participation of his/her officers, employees or volunteers in said activities. The participant named has read this form carefully and is fully aware of the legal onsequences of signing it. The Emerald Coast Parrot Head Club and Fudpucker’s of Destin does not have or provide medical or accident insurance for persons involved in programs sponsored by the Emerald Coast Parrot Head Club and Fudpucker‘s of Destin.

I understand the Emerald Coast Parrot Head Club Chili Cook-Off event is held to celebrate chili, raise money for charity, and recognize our local cooks & vendors. In addition, we expect a large crowd of participants & visitors, but this may be modified due to unexpected situations. Refunds will not be given regardless of weather or unforeseen circumstances that may cause the event to be canceled.
